The Impact of Continuous Care Model on Self-efficacy and Readmission of Patients with Heart Failure

Background: Patients with heart failure (HF) need continuous medical care, including regular follow-up, training, and information acquisition to increase self-care capacity. Adherence behaviors reduces patient readmission Objectives: This study examined the impact of care model (CCM) on self-efficacy patients hospitalized HF. Methods: quasi-experimental was conducted HF visiting two teaching hospitals in southeastern Iran 2021. The participants were 70 who selected using convenience sampling placed into control intervention groups, each 35 through limited random sampling. group received routine but CCM performed for form individual training 6 sessions hospital after discharge then telephone follow-up once a week until end twelfth week. instruments used collect data demographic that assessed their Sullivan's Cardiac Self-Efficacy Scale. collected analyzed SPSS software (version 22) chi-square test, independent paired samples t-test, analysis covariance at significance level P < 0.05. Results: Analysis showed mean score significantly different between groups implementing (P = 0.001). results t-test suggested standard deviation frequencies (1.03 ± 1.01) higher than (0.34 0.68) Conclusions: Given positive significant effect increasing reducing frequency readmissions, considering ease, applicability, low cost this intervention, relevant authorities make effective planning policies implement
